Description

In consideration of participation in/with Prestige Fitness’ exercise program (“Workout"), the undersigned participant ("Participant") and/or the Participant's Parent/Guardian (if the Participant is under 18 years of age), for himself/herself, his/her personal representatives, heirs and next of kin, acknowledges, agrees and represents that he/she has or will inspect the facilities and equipment to be used in the Workout, and if he/she believes that any unsafe conditions exist, he/she will immediately advise a Workout Organizer of such conditions and will refuse to participate until such conditions are corrected.

INDEMNIFICATION AND HOLD HARMLESS

The undersigned (or his/her parent or guardian) hereby agrees to indemnify and save and hold harmless the Workout Organizers and each of them from loss, liability, damage or cost they may incur due to the undersigned's participation in the Workout, whether caused by the negligence of the Workout Organizers or otherwise.

ASSUMPTION OF RISK

The undersigned (or his/her parent or guardian) hereby assumes full responsibility for and risk of bodily injury, death or property damage due to the negligence of the Workout Organizers or otherwise while participating in or observing the Workout.

PUBLICITY

The undersigned (or his/her parent or guardian) hereby consents to the use without compensation, of his/her name and/or likeness, still photographs, video images, biographical material and/or voice in publicity, advertising, and merchandise concerning the Workout, and Workout sponsors and/or their promotion in any and all media and throughout the world.
